Assisted Living and Memory Care are available at Oakmont of Whittier. Medication management, grooming, clothing, bathing, continence care, ambulation, and other services are available in assisted living and memory care.
Our team will supply you with a Physician’s Report as you prepare to move to Oakmont. This form will be used by your primary care physician to give us with any medical information that may aid us in determining how we can best assist you. You will meet with our Health Service Director for a level of care assessment in addition to the Physician’s Report. You and the Health Service Director will review your requirements and develop a care plan during this discussion.
At Oakmont of Whittier, our goal is for our residents to be able to stay with us as their requirements change. Our care plans are adaptable to your specific needs and can be changed at any moment. Our adaptable personal care program can adjust to your changing needs, whether you need more or less care as time goes on.
