Oakmont of Orange provides Assisted Living as well as Memory Care. Medication management, assistance with grooming, dressing, bathing, continence care, ambulation, and other services are available in assisted living and memory care.
As you get ready to move to Oakmont, our team will give you a form called a Physician’s Report. This form will be used by your primary care physician to provide us with any medical information that will assist us in determining how we can best support you. Along with the Physician’s Report, you will meet with our Health Service Director to assess your level of care. You and the Health Service Director will discuss your needs and develop a care plan during this meeting.
At Oakmont of Orange, we want our residents to be able to stay with us as their needs change. Our care plans are adaptable to your needs and can be updated at any time. Whether you require more or less care as time passes, our adaptable personal care program can adapt to your needs.
