Oakmont of El Dorado Hills provides Assisted Living and Memory Care. Medication management, grooming, dressing, bathing, continence care, ambulation, and other services are available in assisted living and memory care.

As you prepare to relocate to Oakmont, our team will provide you with a form known as a Physician’s Report. Your primary care physician will use this form to provide us with any medical information that will assist us in determining how we can best support you. Along with the Physician’s Report, you will meet with our Health Service Director for a level of care assessment. During this meeting, you and the Health Service Director will discuss your needs and develop a care plan.

Our goal at Oakmont of El Dorado Hills is for our residents to be able to continue living with us as their needs change. Our care plans are adaptable to your needs and can be updated as needed. Whether you require more or less care as time passes, our adaptable personal care program can adapt to your changing needs.

Our care team is on-site and available 24 hours a day, 7 days a week. Our licensed nurses are on-site 16 hours a day.
